We are glad you have chosen Cornerstone Pain Management to provide you with pain management and clinical services. We will do our best to treat your medical condition and provide you with quality care.

Prior to making your first appointment with us we do need a doctor to doctor referral. This referral is not from your insurance company but from your primary care doctor or any other doctor you are seeing. Please feel free to print off one of our referral forms for your doctor to make his/her process easier.

Once your appointment is made we want to make the pre-visit process as simple and convenient as possible, so please prepare for this by bringing the following information:


Things you should bring to your first appointment:

  • Health Insurance Card/Information
  • Photo Id
  • A list of all medications you are currently taking
  • A copy of your MRI/Ct scans
  • A completed New Patient Form



Your consultation will last from 30-45 minutes. During the consultation Dr. Sardar will discuss your condition and determine a treatment plan if one is indicated. We will call you to confirm your appointment two business days prior to your visit. If you are unable to answer the phone and you have an answer machine then a message will be left regarding your appointment time and date. Upon new patient consultations please arrive in the office 15minues prior to your appointment. There will be a few remaining in office preparations to be completed prior to you being seen and they need to be finalized by your scheduled appointment time. Sorry for the inconvenience but to space constraint, only one other person will be allowed in the exam room with the patient during the patient /physician examination.

If at any time you are unable to make your appointment please notify us 24 business hours in advance to avoid a no show fee of $30.00. We will be glad to reschedule your appointment at a more favorable time.

It is our policy to refill prescription only during our clinical hours. We require 72 hours on all refill requests. To request a refill please contact your current pharmacy and have them fax over a refill request. Unless it is a monthly prescription you pick up, then please leave a detailed message with name, phone number and prescription on our nurse line before 2 pm. Please take into consideration that we do not refill prescriptions earlier then indication on you current prescription, they must last 30 days. If at all you have any trouble with your medications please contact our office and leave a message on our nurse line or email us at and we will contact you in the order we receive you call/email.
